Your restaurant becomes non-compliant
Health codes require at least one CFPM on-site during operating hours. Expiration = the restaurant is out of compliance.
Cited during inspections; some jurisdictions post 'no manager on duty' notices publicly.
Fines and permit reviews
Fines: $250–$2,500 per violation. Repeated violations trigger permit review or suspension.
In severe cases, health departments have shut down establishments until a CFPM is available.
Different from food handler cards
Food handler cards (line workers) are separate from CFPM (managers).

Renewal requires re-taking the exam
ServSafe does not offer a shortcut renewal. Passing the exam again (proctored, in-person or online) is required every 5 years.

Alternative credentialing options
National Restaurant Association ServSafe is the most recognized, but ANSI-accredited alternatives (Prometric, 360training, StateFoodSafety) are accepted in most jurisdictions.

Franchisors typically require ServSafe specifically. Substituting alternatives can violate franchise agreements.
