What happens if your DOT medical card expires?
The DOT medical card (Medical Examiner's Certificate, Form MCSA-5876) is the health verification behind every CDL. It expires every 12–24 months on its own cycle — independent of your license — and its expiration ends commercial driving instantly.

Your CDL is downgraded automatically
Most states auto-downgrade a CDL to Class D within 60 days of medical card expiration. You keep driving privileges for a regular car — not a commercial vehicle.
The FMCSA CDLIS system flags the downgrade in real time.
Driving commercially anyway is a federal violation
Fines: up to $2,750 for the driver, up to $16,000 for the carrier.
A 'medical card violation' on your CSA record follows you across every future job.
Renewing requires an in-person DOT physical
Only Certified Medical Examiners (registered on the National Registry) can issue MCSA-5876 forms.
Cost: $75–$200. Book at DOT physical clinics, urgent care centers, or occupational health offices.
You must submit the new card to your state DMV
Passing the physical doesn't restore your CDL automatically. You must submit the medical card copy to your state's CDL office within a set window (usually 15 days).
Some states now accept e-submission from the Medical Examiner directly.
Common conditions require shorter cycles
Certain health conditions (hypertension, diabetes, sleep apnea) result in 1-year or 3-month medical cards instead of 2-year.
The examiner sets the cycle at each exam — check your card's expiration every time.
Passenger, school bus, HazMat drivers get scrutiny
Higher-risk endorsements (P, S, H) often have stricter medical review. Any medical card lapse cascades to endorsement suspension.
Some employers require a passing DOT physical every 12 months regardless of federal minimums.
VaultGuardian tracks DOT medical card, CDL expiration, and each endorsement separately — reminding you before an automatic CDL downgrade ends your workweek.
