What happens if your DEA registration expires?

Zero grace period — this is not a state license
The DEA policy is explicit: no grace period. Prescribing on an expired DEA is unlawful distribution.
Federal penalties for unauthorized distribution range from $10,000 civil fines to felony prosecution.
You have to close and re-open
Lapsed DEA registrations cannot be renewed — they must be re-applied for. The re-application takes 30–60 days.
During re-application, you have no controlled substance authority — meaning practice partners must cover all your scheduled prescriptions.
DEA and state controlled substance registrations are separate
Many states require a separate state Controlled Substance Registration (CSR) in addition to federal DEA. Both must be current independently.
Losing either ends controlled prescribing in that state.
Pharmacies verify in real-time
Every controlled prescription runs through state PDMP (Prescription Drug Monitoring Program) systems that verify DEA validity instantly.
Denied prescriptions get reported to the state board and often to the DEA.
Renewal window is 60 days before expiration
DEA opens online renewal 60 days before expiration. Renewing early costs nothing and is the only way to avoid catastrophic lapses.

Notify colleagues and the pharmacy
If you know a lapse is coming, notify your partners, hospital admin, and preferred pharmacies in writing.
This lets others cover controlled prescriptions until you're re-registered — and creates a paper trail proving you didn't attempt unauthorized prescribing.
